Do you have a newborn or are you moving to the area?

We serve patients from birth to 16 years old and are accepting new patients.

 

To register with our office as a new patient, please call us at (770)-991-2176 and select option 1 for Scheduling.

 

Let the receptionist know that you are wanting to become a new patient.   

You will need to make arrangements for your child’s previous medical records to be sent to us.

When transferring your child’s care from another practice, please fill out a medical records release form at your prior office and request they send the records to us

via fax, 770-629-2703 or email, medicalrecords@sappamedical.com.

We must have all previous medical records prior to registering and scheduling your child/children.

Once we receive full medical records, we will contact you to register and schedule your child.

If your child is a new patient and has special health needs, please let our staff know so that our team can assist with planning for the visit.
